RESUMO

Radiation sterilization eliminates microbiological infections but causes the degradation of the cell factor. The negative result of microbiological examination for tissue transplants is one of the conditions for approval for distribution in patients. The study attempts to verify impact of the presence of microbes onto material for transplant loss. In the 2011-2015 period, we analyzed 293 donors of skin and amnion. Microbiological sampling was performed. The total of 21 strains of bacteria, molds and fungi was identified in collected tissue. The widest spectrum of strains was found in skin (17), followed by amnia (8). The total number of positive findings was 147 and was again highest in skin (129), while the number of positive findings in amnia was 18 only. The general percentage of fungal infections was very low. The presence of fungal strains was only observed in allogeneic skin (2%). Large number of microorganisms isolated from the skin before sterilization was observed, so it seems impossible to use allogeneic intravital skin. However, the intravital application of allogeneic amnion obtained from cesarean section remains to be considered.

Bacteriological studies were performed on 189 patients treated during 1985-1989. 800 samples were tested which were taken from various sites of burn wound (3-5 smears from a patient). Surface of burn amounted to 15-86% and degree of burn was IIb/III. In 772 samples, 2073 bacterial strains were found. Most frequent were Pseudomonas aeruginosa (31%) and Staphylococcus aureus (29%). Remaining strains consisted chiefly of Proteus mirabilis, Pseudomonas cepacia, Klebsiella pneumoniae, Escherichia coli and Acinetobacter calcoaceticus. Serological typing of Pseudomonas aeruginosa carried according to Fisher demonstrated dominance of immunotype T3.7 (43%). Staphylococcal strains were most frequently sensitive to group III phages and/or supplementary phages.

RESUMO

Sheep anti-Pseudomonas immunoglobulin and Pseudomonas vaccine were used for combined immunotherapy in 55 severely burned patients. The results of clinical investigations show that active and passive immunization in the early state of thermal injury increases the anti-Pseudomonas agglutinin titer, diminish the threat of Pseudomonas sepsis and death and significantly accelerate the recovery.

RESUMO

Preliminary clinical evaluation of sheep anti-Pseudomonas immunoglobulin was performed on 20 burn injured patients. The patients were divided into two comparable groups of ten persons. One group was treated with the immunoglobulin shortly after thermal injury (in about two days). The patients of the second group received this preparation in about 18 days post thermal injury. It was concluded that the sheep anti-Pseudomonas immunoglobulin is well tolerated by patients. The results of clinical observations show that, in respect to incidence of sepsis and survival, only early administration of immunoglobulin can prevent further development of infection caused by Pseudomonas aeruginosa.
